Easily Access Your Child’s Treatment Notes Online

With the Clubhouse Parent Portal, you can now access your child’s therapy notes, view previous and upcoming appointments, and upload documents directly through the link below! For first-time users, follow the login instructions on this page.

Logging Into the Parent Portal

• Let your child’s therapist know that you’d like access to their records to ensure that they have been shared by the therapist.

• On your web browser, navigate to: https://members.centralreach.com

If you are logging in for the first time, select “forgot password” enter your email address, and follow the provided link to reset your password. Remember to use the same email address you have provided to the Clubhouse for this step.

• Once reset, you can log in and will have access to your child’s therapy information.

**If you are unsure which email address you provided to the Clubhouse, please ask our admin team**

About the Parent Portal

• If you need to reschedule an appointment, please contact our admin team. Please do not use the Parent Portal to reschedule or cancel your appointment.

• Clicking on the “Files” button will allow you to see all of the documents that your child’s therapists have made available. This should include all current and future treatment notes and reports. To read a daily note or other document, click on it and either download or preview the document.

• You can also upload documents to the portal rather than emailing, faxing, or filling out a paper copy in the office. To do this:

  1. Click the “Files”  button and select “Upload New File”
  2. Drag your document or click to browse and select the file you wish to upload.
  3. Your document will appear in the list of uploads after you refresh your screen.
