Helping children improve is often a group effort! Extended family members, caregivers, Day Care facilities, etc. may be an integral part of the therapy process.
By signing below, I am granting permission to the Clubhouse Staff to share therapy notes and information regarding therapy progress with persons that bring my child to therapy (nanny, Grandparent, etc.). In the event that my child is seen at a day care facility or babysitting site, the therapy staff may share therapy suggestions with that caregiver as well.
I verify that I am the legal custodian of this child and have the legal right to make decisions regarding his/her medical care.
